Application form must be completed and submitted two ways:

- By email, as PDF
Your application must contain all relevant documents (including additional pages needed to complete any of the questions, your letter and proof of legal status in Canada). Letter of support may be emailed or mailed directly to the CSI Research Foundation.
Incomplete applications will not be considered.
